About the Position

At 21st Century Care Solutions our Aging Life Care Managers, also known as geriatric care managers, are professionals who provide support to our clients and their families. The services provided include a range of direct and indirect supports. The Aging Life Care Manager will: coordinate the creation and implementation of a bio-psycho-social care plan, navigate health care services, assist with decisions such as ageing in place or relocation to a residential facility etc.

The Aging Life Care Manager can be the central communication point for communication and coordination between the client, the family and an array of health care and other lo service providers.

Since our company also provides full fiduciary services and financial management, the Aging Life Care Manager will also support the Licensed Fiduciary to secure and maintain public benefits, and support the overall plan for managing and allocating the client’s finances and property for both voluntary and court managed cases such as Conservatorships and Special Needs Trusts.

21st Century Care Solutions provides wrap-around Fiduciary Services and Aging Life Care Management using a holistic, client-centered approach to caring for older adults and adults with special needs who face ongoing challenges.

Position Description

Essential Duties and Responsibilities include the following. Other duties may be assigned.

· Consults with clients and families to address healthcare needs.

· Serves as advocate at medical and other appointments.

· Upholds a client-centered approach in all areas of interaction with and about the client’s welfare and wellbeing.

· Assesses and monitors psychosocial and physical needs and improves overall client care.

· Creates a customized, comprehensive Plan of Care.

· Facilitates client-family communication and relationships.

· Acts as a liaison to families at a distance and promptly alerting families to problems or concerns.

· Arranges and coordinates in-home and other services or resources.

· Addresses household and financial issues with attention to conserving financial resources.

· Assists with coordinating client moves to or from independent housing, retirement community, assisted living community, or skilled nursing facility.

· Works closely with our tight-knit team, committed to ongoing process improvement based on client relationships and feedback.

· Participates in professional development and keeps all licenses and certifications current.

· Uses electronic record-keeping system to keep confidential client records.

· An excellent communicator, both verbally and in writing

· Independent, willing to learn, flexible, and quick to adapt

Corporate Compliance

· Complies with safety policies and procedures

· Complies with all aspects of HIPAA

· Adheres to laws on the protection, use, disclosure, and release of client information

· Adhere to department COVID-19 safety protocols for staff and client interactions

· Completes continuing education requirements, maintains all professional licenses

· Reports violations of law, regulations, policies, or procedures

· Reports suspected client abuse or neglect

· Records documentation timely and accurately

Qualifications

· Ability to travel within our service radius and work with clients in their homes, facilities and with medical providers.

· Valid California driver's license, up-to-date auto insurance and vehicle available for work.

· Willing to undergo a hiring background check, fingerprinting, TB Test, and Drug Test.

· Ability to use/willingness to learn technology, including virtual platforms, various software, computer, and smartphone.

Education and/or Experience

Preferred: MSW or Masters in Gerontology

Minimum: Will also consider RN, PT, OT, BSN, or MA/BA/BS degree in social science, social work, or related field with relevant experience with seniors or individuals with special needs.

Relevant Years of Experience

Preferred: 3 years

Minimum: 1 year of relevant experience with seniors or individuals with special needs.

Job Type: Part-time

Pay: $40.00 - $65.00 per hour
